Hip Osteoarthritis

Also known as: Hip OA، Coxarthrosis، Degenerative hip disease

Hip osteoarthritis is gradual wear of the cartilage in the hip joint, leading to pain, stiffness, and reduced mobility. Less common than knee OA but typically more disabling — it affects walking, sitting, sleeping, and daily activities.

Common Symptoms

  • Deep groin pain (most characteristic — not always 'in the hip')
  • Pain that radiates to the thigh, knee, or buttock
  • Stiffness after sitting or in the morning
  • Limp when walking, especially distance
  • Difficulty putting on shoes or socks
  • Pain getting in and out of cars
  • Pain at night, sleeping on the affected side
  • Reduced range of motion — difficulty crossing legs

Causes & Risk Factors

  • Age over 50 (most common factor)
  • Hip dysplasia from childhood (often undiagnosed)
  • Previous hip injury or fracture
  • Avascular necrosis of the hip (blood supply problem)
  • Repetitive heavy use occupations
  • Obesity
  • Genetic predisposition
  • Other joint conditions (rheumatoid arthritis)

Treatment Options

Physical therapy and weight management

Strengthening the hip and core muscles reduces joint load. For overweight patients, weight loss produces dramatic pain improvement. First-line treatment.

Low-impact exercise

Swimming, cycling, and elliptical maintain mobility without joint stress. Walking is also beneficial when not too painful. Avoid running, jumping sports, deep squats.

NSAIDs and analgesics

Short courses for flare-ups. Best used short-term due to gastrointestinal and cardiovascular risks. Topical NSAIDs less effective for deep hip joint than for knee.

Intra-articular injections

Hyaluronic acid or PRP injections under ultrasound or fluoroscopy guidance. Less proven for hip than for knee, but useful in some patients to delay surgery.

Hip arthroscopy (selected cases)

For specific intra-articular pathology (labral tears, FAI). Not effective for advanced osteoarthritis. Used in younger patients with early hip problems.

Total hip replacement

The definitive treatment for advanced hip OA. Highly successful — 95% of patients experience dramatic pain relief. Recovery: walker first week, cane by week 2-3, unassisted by week 4-6. Most modern implants last 20+ years.

When to See a Doctor

  • Hip or groin pain persisting more than 6 weeks despite rest
  • Limp lasting more than a few days
  • Pain preventing normal walking, sleeping, or daily activities
  • Pain after a fall (especially in older patients — rule out fracture)
  • Difficulty putting on shoes or socks
  • Sudden severe hip pain in a child or young adult (consider Perthes, slipped epiphysis)
